Applications are invited from the citizens of India who are native of Tamil Nadu
for admission to MBBS / BDS Degree Course for 2012-13 session in Govt. /
Self-Financing Medical / Dental Institutions coming under the purview of Single
Window System (SWC) in the State of Tamil Nadu.

Application Cost:
Rs.500/- (General); Rs.100/- (for each Special Category - Physically Handicapped
/ Children of Freedom Fighters / Children of Ex-Servicemen / Eminent Sports
persons) and SC / ST candidates are exempted from payment of cost of application
and Prospectus. They can obtain the application on written requisition duly
enclosing two sets of attested Xerox copy of the Community Certificate and a
copy of Plus Two Mark Sheet.
Mode of Payment (DD):
Demand Draft in favour of "The Secretary, Selection Committee, Kilpauk, Chennai
- 600 010" payable at Chennai from any Nationalised bank. The name and address
of the candidate should be written in the reverse of the demand draft.

Applications forms will NOT be issued from the Directorate of Medical
Education and Office of the Selection Committee, Kilpauk, Chennai - 10.
The candidates can obtain the application on a written requisition lo the
Deans / Principal of the above Government Medical / Dental Institutions duly
enclosing a crossed Demand Draft from any Nationalized Bank dated not
earlier than 14.05.2011 payable at Chennai towards the cost of application
form with Prospectus.

Application forms can also be downloaded from www.tnhealth.org, www.tn.gov.in and
send the application along with the crossed Demand Draft. (Rs.500/- or
Rs.100/- as the case may be). SC/ST candidates need not enclose Demand
Draft.
Candidates applying for Special Category Form I should enclose the prescribed
Special Category format duly filled and enclose the same along with common
application in the same cover. They should also enclose a Demand Draft for
Rs.100/- drawn in favour of ” The Secretary, Selection Committee, Kilpauk,
Chennai – 10 ” payable at ” Chennai ” prescribed for each Special Category
format should not be sent separately. Candidates shall apply to a maximum of
three Special Category with relevant documents.
Candidates applying under “Orthopaedically Physically Disabled” Category have to
enclose the certificate as specified in the prospectus only. This certificate
must have been obtained within three months prior to submitting the application.
The completed application forms should reach "The Secretary, Selection
Committee, No.162, Periyar E.V.R High Road, Kilpauk, Chennai - 600 010" on or
before 02.06.2012, by 05:00 PM.
