Required Documents:
- Recent passport size photograph
- Address proof (Aadhaar, Ration Card, Voter ID, etc.)
- Birth Certificate
- School Education Certificate or Employment Details or other proof to confirm continuous residence of last 5 years
- Self-declaration of applicant
