How to retrieve lost ration card?

Who to approach - The food items distribution officer in villages and food items supply and distribution Assistant regional collector in the cities
What are the documents requires- A photocopy of the lost ration card or any other personal identity proof.
What are the charges- You have to Pay Rs. 10/- while receiving the new ration card.
Time limit - Issue of new ration card within 45 days after the receipt of the application for the new one
Procedure - You have to submit aletter intimating the loss of the ration card to the official and fill up the application given by them. After their icheck/investigation, new card will be issued.
