Persons, who are tested for COVID-19 in Chennai, shall mandatorily fill up this form of self-dedication of contact details at the time of giving a sample, which will help the Greater Chennai Corporation (GCC) to reach out to them for timely care and assistance.
It is mandatory for all authorized labs to send this form to GCC after getting filled by every person, from whom the test samples for COVID-19 was taken.
